You can get the form at the Chinese embassy, maybe online too. If the school says they just need a doctors note then maybe that will do. The Embassy will only need to see the letter of invitation, so as long as the school can get you that it will be fine.

Having said that, often the schools say one thing and then find out that they are wrong. If it is easy to do I would go for the correct form!
