It's, I think, a matter of development level, and China is still a developing country (that's what the Chinese government claims officially).

In less developed nations, they are simply much less aware of the need for public hygiene and the problems of infection if the latter is lacking. It's ignorance, not knowing and a disinterest in things like that.
